First Time Applicants

Before You Apply for Your First NAC Grant:

1. Confirm Nonprofit Status
To be eligible for an NAC grant, your organization must be a federally tax exempt organization (usually with 501(c)(3) nonprofit status) as defined
• by the IRS: verify your IRS status at www.guidestar.com.
• with the State of Nebraska: verify your State of NE status at www.sos.state.ne.us.

Exceptions:
Schools, churches or divisions of government must provide a copy of your organization’s completed Sales Tax Exemption Certificate.

2. Mail in a completed W9 Form to the NAC at 1004 Farnam St., Lower Plaza, Omaha, NE 68102.

3. Mail in a completed ACH Form to the NAC at 1004 Farnam St., Lower Plaza, Omaha, NE 68102.

An ACH Form is required for receiving payments via direct deposit to your organization’s bank account. (If your organization already receives electronic payments from the State of Nebraska, you do not need to complete this form. Please let us know if this is the case).
*Please note: The NAC will be unable to process grant payments to your organization until these materials are submitted.

4. Get a D-U-N-S number
The NAC is following the lead of the National Endowment for the Arts and now requires organizations to provide a D-U-N-S number as part of their applications:
